From a statistical perspective, it is almost inevitable that some schools, church groups, friendship circles, and neighborhoods will be associated with clusters of chronic diseases. When first noticed, such clusters are often regarded as resulting from some specific, predictable process, rather than as events with independent causes that happened to have occurred by chance in one particular place (such as a coin toss). A "cluster" is an unusual aggregation, real or perceived, of health events that are grouped together in time and space and that is reported to a public health department (CDC 1990). Several breakthroughs and triumphs in infectious disease control have resulted from the epidemiologic evaluation of clusters of cases. Well-known examples of clusters include the epidemic of cholera in London in the 1850s (Snow 1965), the investigation of cases of pneumonia at the Bellevue-Stratford Hotel in Philadelphia in 1976 (Fraser et al. 1977), and the 1981 report that seven cases of Pneumocystis carinii pneumonia had occurred among young homosexual men in Los Angeles (CDC 1981).

To address basic measurement issues, including those of test sensitivity, specificity, and reliability. To describe and discuss factors that influence olfactory function, including airborne toxins commonly found in industrial settings. METHODS: Selective review and discussion. RESULTS: A number of well-validated practical threshold and suprathreshold tests are available for assessing smell function. The reliability, sensitivity, and specificity of such techniques vary, being influenced by such factors as test length and type. Numerous subject factors, including age, sex, health, medications, and exposure to environmental toxins, particularly heavy metals, influence the ability to smell. CONCLUSIONS: Modern advances in technology, in conjunction with better occupational medicine practices, now make it possible to reliably monitor and limit occupational exposures to hazardous chemicals and their potential adverse influences on the sense of smell. Quantitative olfactory testing is critical to establish the presence or absence of such adverse influences, as well as to (a) detect malingering, (b) establish disability compensation, and (c) monitor function over time. Itch (pruritus) is ‘an unpleasant cutaneous sensation which provokes the desire to scratch’.1 Like pain, acute itch serves a protective function, but chronic itch is mostly a nuisance. Body Temperature It is important to maintain a constant temperature so that living organisms can maintain metabolism. There are two types of heat regulation: endothermic where the species controls their own temperature (mammals, birds), and ectothermic where temperature reflects the environmental temperature (lizards, fish). Temperature in mammals is detected by thermoreceptors in the skin and hypothalymus which is in the brain. Changes in temperature bring about nerve impulses from the brain to the muscles and glands which will bring about changes depending on whether it is hot or cold.
